Please click on the permission slip for the activity that your child will be attending. 

Print the form, fill out the bottom portion (contact information for that day) and sign.

Bring the completed form on the date of the event but please don't count on extra copies to fill out when you arrive. You may also return a copy to Sherri via fax 770-807-6468 or via scan/email prior to the event.  

We will try to include on the form any additional items the child will need to bring, but please note the standard items for outdoor events: water bottle, tennis shoes, hat.
